"Fighting On" Deciding how and when to continue care at the end of life

Synopsis: 

Fighting on gives a real-life account of the challenges that come with end of life care.  Should care be prolonged?  How do you communicate your wishes as a patient, care giver, or Doctor?  It also touches on the important role of the primary care physician throughout these difficult decisions.
